Please note that custom URLs will no longer work once the username is changed, so if you are currently using any, you will need to set it up again.

I would like to request to change the user name of the Jotform.

Change: Yebin Cho

Reason: Due to the resignation of the existing person in charge, I would like to request you to change the name of the account using the Jotform.
